PATIENT ELIGIBILITY

To be eligible for Upper Chesapeake Hospice, a patient must:

be in an advanced stage of a life-limiting illness;
be under the care of a physician who understands and will
participate in the hospice concept of care.

Charges to the patient will depend on ability to pay.
